No. 5182, Ultrathin Bonded Asphalt Overlay from KTA Milepost 96.3 to KTA
Milepost 112.3 are amended in the following particulars only, all other terms
and conditions remaining unchanged:

ADD: “SPECIAL INSTRUCTIONS &
INFORMATION, KTA CONTRACT NO. 5182, REVISED”, Rev. 11/09, enclosed herewith.
This Addendum is hereby made part of
the Contract Documents to the same extent as if contained in the original
Contract Documents.
Each bidder shall acknowledge receipt
of this Addendum in the space provided below and shall include this Addendum as
part of the Proposal.

SPECIAL INSTRUCTIONS &
INFORMATION
KTA CONTRACT NO. 5182
NOTE: Whenever this Special Provision conflicts with the Plans,
Supplemental Specifications, or Standard Specifications, this Special Provision
shall govern.
1.0 DESCRIPTION:
The work under this contract will include 5/8 inch ultrathin
bonded asphalt overlay from Milepost 96.3 to 112.3.
There shall be no profilograph requirement on this project.
Any milled material shall remain the property of this KTA.
The Contractor shall apply SS-1H tack oil on the milled
median shoulder prior to asphalt placement.
Shoulder work shall be done in a manner so there is no tracking of tack
onto the new roadway surfaces.
2.0 TRAFFIC CONTROL:
Each operation of work, roadway planing, asphalt, and
pavement markings, shall have its own separate traffic control. This means each operation will have separate
arrow boards, signs, and barrels.
The Contractor will be allowed no more than five (5) miles
of construction zone requiring a lane closure in any one direction of
travel. In no case shall a lane be
closed in one direction for more than five (5) miles.
If the Contractor has no activity in a work zone for a
period exceeding one (1) day, the Engineer may require removal of non-essential
traffic control devices in the interest of keeping as much of the roadway open
as possible.
